• Personal Data Sheet (PDF)
• Intern Biography Sample (PDF)
• W-9: tax form for US citizens (PDF)
• W-8: tax form for foreign citizens (PDF)
• A scanned copy of your passport and visa (foreign citizens only)
• I-94 and I-20 or DS-2019 (for foreign citizens only)
